North Carolina doesn't have a numbered bill of sale form — a plain bill of sale is used as supporting paperwork, but MVR-1 (Title Application) is the required, notarized document that actually transfers the title.

Bring the unsigned title and a blank MVR-1 to your notary appointment — signing early can void the notarization.

Vehicle description, VIN, and buyer/seller information.

Sign both the title and MVR-1 in front of a notary
North Carolina requires notarized signatures for both documents.

Write up a separate bill of sale
Include sale price and date — NC DMV requests this alongside MVR-1 at registration.

File within 28 days
Bring the notarized title, notarized MVR-1, bill of sale, and proof of NC insurance (FS-1) to an NCDMV license plate agency. Pay the $56 title fee plus 3% Highway Use Tax.
